This form MUST be filled in EVERY time there is a violent or aggressive incident, whether you are injured or not.
This form is usually submitted for violence or harassment between employees.

MUST be filled in for every violent or aggressive incident – whether you are injured or not.
This is to be filled in whether you are the victim of any of the incidents listed on the form or witness an incident from a student to a co-worker or another student. The incidents to be included according to Board policy are as follows:
a) uttering a threat to inflict serious bodily harm on another person
b) possessing alcohol or illegal drugs
c) being under the influence of alcohol
d) swearing at a teacher or at another person in a position of authority
e) committing an act of vandalism that causes extensive damage to school or Board property or property located on school or Board premises;
g) bullying – previous suspension/risk to others
h) motivated by prejudice/bias/hate
i) habitual neglect of duty
This is the ‘receipt’ you should receive back from your Principal.
This MUST be filled in by the Principal every time you get hurt at work AND take time off OR seek medical attention due to these injuries.
